To certify for the Adult Eye Care program, please contact Karen
at 376-4571 to schedule an appointment.  Appointments times are available Tuesday through Thursday from 8:30-11:30 am,  and
1:00-3:30 pm. The Adult Eye Care Program works in conjunction with local optometrists and ophthalmologists to provide eye exams and glasses for low-income adults. The eye care professionals provide all of their services free of charge.In order to qualify for a prescription and eye glasses a person must:

  • Be employed, receive Social Security/veterans benefits or be a full time student.
  • Have proof of residency (Provide a current rental agreement or a utility bill with name and address.)

Individuals seeking assistance from the Adult Eye Care program may fill out an application at our office. Once approved, an appointment is made for an eye exam with one of our participating doctors. Next, they take their prescription to our office to pick out a pair of frames and receive a voucher for lenses all at no cost to the participant.
