Eye Care Services
Free of Charge!
The Adult Eye Care Program works in conjunction with local optometrists and ophthalmologists to provide eye exams and glasses for low-income adults. The eye care professionals provide all of their services free for our clients as the costs are covered by the Family Care Foundation.
Individuals seeking assistance from the Adult Eye Care program may fill out an application at our office located at 1900 N Spring Street. Once approved, an appointment is made for an eye exam with one of our participating doctors.
Program Participant Qualifications
In order to qualify for a prescription and eyeglasses a person must:
Be employed (cannot earn more than $2,383/month per household)
If unemployed, be actively looking for employment and applying for jobs
Receive Social Security/veterans benefits
Have proof of residency (provide a current rental agreement or a utility bill with name and address)
